Connecticut Renews the Fight in Chronic Disease

Please join us in welcoming Ken Thorpe, Ph.D. Chairman of the Partnership to Fight Chronic Disease and co-director of Emory Center on Health Outcomes and Quality as he updates us on ongoing national efforts to address disease prevention and the treatment of chronic disease in the new healthcare exchanges.

Join us as we discuss with Ken the ways that chronic disease needs to be addressed in a more patient centered way within the new healthcare landscape. Including ensuring that patients do not have to choose their new health plans based on which of their conditions is either the most severe or the most expensive.

Prevention of chronic illness and access to consistent treatment still remains one of the best methods of fighting off disease and keeping patients out of the hospitals.
